I confim and agree to reactivate my membership with Victorian YMCA Community Programming Pty Ltd on the terms of my membership contract and the direct debit arrangements in place prior to my membership being suspended
I understand that access to utilise the pool outside of my lesson will require an online booking before attending the centre and that services at the centre and my access to the centre may be restricted or limited due to the COVID 19 pandemic. I understand this may impact my ability to access or attend the centre and that Victorian YMCA Community Programming Pty Ltd. may be required to cancel or amend lesson days and times based on circumstances beyond our control. This will be communicated to you at the time.
I understand that, if I am not the bank account holder for my membership (i.e. another person pays for my membership), Victorian YMCA Community Programming Pty Ltd will contact the bank account holder to confirm that they provide consent for the membership debit to recommence. I acknowledge and understand that if the bank account holders' consent is not provided, the membership will not be reactivated. (Note that Victorian YMCA Community Programming Pty Ltd will contact you in these circumstances to confirm whether you wish to provide alternate bank account details via a new direct debit request form)
For parents / guardians only: I confirm that I am the parent or guardian of the member and that I have the authority and approval of the member to reactivate the membership on the terms of the membership contract and the direct debit arrangements in place prior to the membership being suspended. I also confirm that I will pay the membership fees based on those direct debit arrangements once the membership is reactivated
I understand that Federal and State Government orders, announcements and guidelines continue to change and that my membership may need to be further suspended if Victorian YMCA Community Programming Pty Ltd determines this appropriate or necessary for the health and safety of centre staff and patrons. Victorian YMCA Community Programming Pty Ltd will contact you in these circumstances.
